For your private insurances

For a life insurance, contact AG directly.
Make sure you have a copy of the death certificate and your ID card.

In case of disability or work incapacity insurance: download the corresponding claim notification/medical certificate form. Complete the document and send it by email to claims@aginsurance.be or by post to AG Insurance, Claims/1JQ3F, Boulevard É. Jacqmain 53, 1000 Brussels.
